What this book does for you
Grad school taught you how to help people. It skipped the part about getting paid for it.
This book covers that part. You’ll figure out what an hour of your time really costs, set a fee you can say out loud without flinching, and keep more of every session you see. You’ll do it the right way, with the California paperwork, the ethics codes, and the tax side squared away.
It’s a 6 CE hour book. $19 for 6 to 7 credit hours. Read it, pass the post-test, and the hours count toward your renewal.
Who this book is for
- LMFTs, LCSWs, and LPCCs who want to start or grow a cash-pay practice
- Clinicians on insurance panels or platforms who want to know what leaving would really take
- Associates who want to learn the business side now, under California’s rules for associates
- Anyone who would rather start with one cash client than bet the farm
Level: Intermediate
What you get
- The book Get Paid What You're Worth: Building a Cash-Pay Practice Legally and Ethically by California Counselor CEUs (John Oliver Ycaro, LPCC) (PDF download, in full color and a print-friendly black-and-white version)
- The Cash-Pay Practice Ledger, a ready-made spreadsheet with tabs for income, expenses, a monthly summary, tax set-aside, monthly reconciling, and mileage. The formulas are already built. It opens in Excel, Google Sheets, Numbers, or LibreOffice.
- Two “Tools That Grow With You” lists, one for the back office and one for marketing. Each shows a free way to start, a low-cost step up, and what to grow into, checked on each vendor’s own page in October 2026.
- A 38-question online post-test (70% to pass)
- 6 CE hours and a certificate emailed to you within 1 business day after you pass the post-test and complete the evaluation
Instructor
John Oliver Ycaro wrote this book and teaches this course. He is an LPCC who worked full time as a clinician at the VA while running a private practice on the side. He wrote this book to put the numbers, the paperwork, and the ethics in one place, in plain language, so you can charge a fair fee and build your practice the right way.
Learning objectives
After completing this course, you will be able to:
- Explain what the ethics codes say about fees and fee disclosure, and describe how to decline a client who can’t afford your fee without pressure, with referrals to lower-cost care.
- Calculate your real hourly rate and monthly need, and use them to set a session fee.
- List the California legal and ethical setup steps for a cash-pay practice, including the business license, NPI, HIPAA and California privacy law, fee disclosure, the Good Faith Estimate, superbills, records, and advertising rules.
- Describe an ethical order of steps for leaving insurance panels and platforms, including client notice, non-abandonment, and referrals.
- Set up and keep a practice ledger, reconcile it each month, and plan quarterly estimated tax payments.
- Use caseload math and ethical marketing to grow a practice, including raising fees with notice and adding lower-fee spots by choice.
